If you have been researching weight loss medications in Canada, you have probably come across Wegovy. It made headlines as a "game changer" and for good reason. But there is a lot of confusion about what Wegovy actually is, how it differs from Ozempic, and whether you can even get it in Canada right now. I wrote this guide to cut through the noise and give you the real picture based on clinical data and the Canadian healthcare landscape.

Wegovy is the weight loss version of semaglutide. Same molecule as Ozempic, different dose, different approval. What Makes Wegovy Different From Ozempic?

This is the most common question I get, and the answer is simpler than you might think. Wegovy and Ozempic contain the exact same drug: semaglutide, made by Novo Nordisk. The differences come down to three things:

Dose: Ozempic tops out at 2 mg per week. Wegovy goes up to 2.4 mg per week, which is the dose studied in the major weight loss trials.

Approved indication: Ozempic is approved by Health Canada for type 2 diabetes. Wegovy is approved for chronic weight management. This distinction matters for insurance coverage and how your doctor prescribes it.

Titration schedule: Wegovy uses a 5-step dose escalation over 16 weeks. Ozempic has a simpler 3-step titration since the target doses are lower.

From a molecular standpoint, there is zero difference. Your body cannot tell them apart. The reason Novo Nordisk sells them as separate products is regulatory and commercial. Because Ozempic is widely prescribed off-label for weight loss with better insurance coverage for diabetes, some Canadian doctors prescribe Ozempic at higher doses as a workaround during Wegovy supply shortages.

The STEP Trial Results: Real Weight Loss Numbers

Wegovy's approval was based on the STEP (Semaglutide Treatment Effect in People with obesity) trial program. These were large, randomized, double-blind, placebo-controlled trials. Here are the headline numbers:

In STEP 1, the average starting weight was about 105 kg (231 lbs). A 14.9% loss means roughly 15.6 kg (34.4 lbs) over 68 weeks. About 32% of participants lost 20% or more of their body weight, while some lost less than 5%. STEP 3 combined Wegovy with intensive behavioural therapy and achieved 16% average loss, suggesting lifestyle changes on top of the medication lead to better outcomes.

STEP 4 answered the crucial question: what happens if you stop? After 20 weeks on semaglutide, participants who switched to placebo regained about two-thirds of their weight loss over the following 48 weeks. This tells us that Wegovy is likely a long-term commitment for sustained results.

The SELECT trial (17,604 participants with overweight/obesity and cardiovascular disease but without diabetes) showed that semaglutide 2.4 mg reduced major cardiovascular events by 20%. This was the first time any weight management drug demonstrated direct heart protection.

Wegovy Dosing: The 5-Step Titration Schedule

Wegovy uses a gradual dose escalation to minimize side effects, especially nausea. You do not start at the full dose. Here is the standard schedule:

If you miss a dose by more than 5 days, take it as soon as possible and resume your regular schedule. If more than 5 days have passed, skip that week and take the next scheduled dose. Do not double up.

Who Qualifies for Wegovy in Canada?

Health Canada approved Wegovy for adults (18 years and older) who meet one of these criteria: BMI of 30 or greater (obesity), or BMI of 27 or greater (overweight) with at least one weight-related health condition such as type 2 diabetes, high blood pressure, dyslipidemia, or obstructive sleep apnea.

Wegovy is not appropriate for patients with a personal or family history of medullary thyroid carcinoma, Multiple Endocrine Neoplasia type 2, or a history of pancreatitis. It must be stopped at least 2 months before a planned pregnancy. In practice, most Canadian doctors focus on your metabolic health picture rather than BMI alone.

What to Expect Month by Month

Weeks 1 to 4 (0.25 mg): Most patients notice reduced appetite within the first week or two. Weight loss is modest, typically 1 to 2 kg. Nausea is common but usually mild.

Weeks 5 to 8 (0.5 mg): Appetite suppression becomes more noticeable. You may find you cannot finish meals you used to eat easily. Some patients report changes in food preferences, finding greasy or very sweet foods less appealing.

Weeks 9 to 12 (1.0 mg): This is where many patients see significant momentum. Weight loss typically accelerates. GI side effects may return briefly with each dose increase but tend to settle within 1 to 2 weeks.

Weeks 13 to 16 (1.7 mg): Approaching the maintenance dose range. Most patients have lost 5% to 8% of their starting weight by this point.

Week 17 onward (2.4 mg): The full dose. Weight loss continues gradually. Most of the total weight loss occurs in the first 9 to 12 months. After 12 months, weight tends to plateau and the focus shifts to maintenance.

Individual variation is significant. Some people lose 20% or more of their body weight. Others plateau at 8% to 10%. Response at the 3-month mark is often a good predictor of long-term results.

Side Effects Overview

The most common issues are gastrointestinal: nausea (about 44%), diarrhea (30%), vomiting (24%), and constipation (24%). These are typically worst during dose escalation and improve once you reach your maintenance dose. Serious but rare side effects include pancreatitis, gallbladder problems, and potential thyroid concerns.

Cash retail prices for generic semaglutide at Canadian pharmacies are now coming in below the telehealth alternatives, based on early Canadian consumer reports. Costco Pharmacy is the lowest reliable option at roughly $88 to $99 per month (confirmed pickups: $88.88 GTA, $88 Ontario, $99 Laval, $91 Medicine Hat). Walmart and Loblaws No Frills typically come in around $95 to $110 per month. Shoppers Drug Mart, Rexall and London Drugs are running roughly $100 to $120 per month (one Halifax-area Shoppers fill reported $113 for the 0.25mg starter dose). Apotex's Apo-Semaglutide Injection began shipping to Canadian pharmacies on May 20, 2026, with Dr. Reddy's generic also launching in May 2026.

That makes in-person pharmacies — especially Costco — meaningfully cheaper than telehealth providers for generic semaglutide. Felix Health and Hims Canada both list $149 per month all-in for the same generic Apo-Semaglutide on their public pricing pages. For most Canadians with a valid prescription, walking it into a local pharmacy is now the cheapest reliable path.

Pricing context: per the Globe and Mail, Apotex's published wholesale price is $78.14 for a four-week supply — roughly one-third of brand-name Ozempic's $240.48 wholesale price.
